ABSTRACT

Introducción: La hemorragia digestiva alta tiene una elevada morbimortalidad. La endoscopía digestiva alta es el estudio de elección para su diagnóstico y tratamiento. Objetivo: Describir la conducta ante la hemorragia digestiva alta. Métodos: Para la revisión bibliográfica se consultaron artículos científicos indexados en idioma español e inglés, relacionados con la hemorragia digestiva, publicados en las bases de datos PubMed, SciELO, Medline y Cochrane, pertenecientes a autores dedicados al estudio de este tema. Desarrollo: La hemorragia digestiva alta se clasifica, según la etiología de origen, en variceal y no variceal. La mayoría de los pacientes con hemorragia digestiva alta el sangrado se autolimita. La causa más habitual es la úlcera péptica, pero en caso de sangrado masivo la etiología más frecuente es la variceal. El empleo precoz de la terlipresina en los pacientes con hemorragia digestiva alta variceal mejora el control del sangrado y disminuye la mortalidad. Se debe hacer uso de escalas validadas de estratificación del riesgo: escala de riesgo de Rockall (tiene como propósito principal predecir la mortalidad y riesgo de resangrado del paciente) y la escala de Glasgow-Blatchford). Conclusiones: Sospechar la presencia de hemorragia digestiva alta, estratificar su riesgo e instaurar el manejo inicial y apropiado constituye una prioridad para el médico de urgencia.


Introduction: Upper gastrointestinal bleeding presents high morbidity and mortality. Upper gastrointestinal endoscopy is the study of choice for its diagnosis and treatment. Objective: To describe the management of upper gastrointestinal bleeding. Methods: For the bibliographic review, the consultation was carried out of scientific articles indexed in Spanish and English, related to gastrointestinal bleeding, published in the databases PubMed, SciELO, Medline and Cochrane, belonging to authors dedicated to the study of this subject. Development: Upper gastrointestinal bleeding is classified, according to the etiology of origin, into variceal and nonvariceal. In most patients with upper gastrointestinal bleeding the bleeding as such is self-limiting. The most common cause is peptic ulcer; however, in the case of massive bleeding, the most frequent etiology is variceal. Early use of terlipressin in patients with variceal upper gastrointestinal bleeding improves bleeding control and decreases mortality. Validated risk stratification scales should be used: Rockall risk scale (its main purpose is to predict patient mortality and risk of bleeding recurrence) and the Glasgow-Blatchford scale. Conclusions: Suspecting the presence of upper gastrointestinal bleeding, stratifying its risk, as well as instituting initial and appropriate management, are a priority for the emergency physician.

Introducción: La tomografía de emisión de positrones es una técnica diagnóstica no invasiva que permite tomar imágenes del organismo que muestra el metabolismo de los órganos del cuerpo. Objetivo: Destacar el valor de la PET/CT en el diagnóstico imagenológico prequirúrgico del enfermo. Presentación de caso: Se presentó un paciente masculino de 39 años sin antecedentes de importancia, con un cuadro de hipoglucemias severas de 5 años de evolución, a pesar de los múltiples estudios imagenológicos se incluyó la ecoendoscopía digestiva, lo que no fue posible evidenciar la lesión tumoral. Se le realiza PET/CT cuyo resultado fue crucial para localizar el tumor, se le dio al paciente la oportunidad de un tratamiento quirúrgico y la demostración anatomopatológica de insulinoma. Conclusiones: Los insulinomas son tumores pancreáticos poco frecuentes que provocan hiperinsulinismo endógeno y son difíciles de visualizar debido a su tamaño por las técnicas de imágenes convencionales, por lo que el PET/CT es un estudio bastante efectivo para localizar la lesión tumoral, y así realizar un procedimiento quirúrgico.


Introduction: Positron emission tomography is a non-invasive diagnostic technique, allowing images of the body to be taken that show the metabolism of the body's organs. Objective: To highlight the value of PET/CT in the pre-surgical imaging diagnosis of the patient. Case presentation: We report the case of a 39-year-old male patient with no significant medical history, but a 5-year history of severe hypoglycemia. Despite multiple imaging studies, digestive ultrasound endoscopy was included, which was not possible to demonstrate the tumor lesion. PET/CT was performed, the result of which was crucial in locating the tumor. The patient was given the opportunity for surgical treatment and the pathological demonstration of insulinoma. Conclusions: Insulinomas are rare pancreatic tumors that cause endogenous hyperinsulinism and are difficult to visualize due to their size using conventional imaging techniques, therefore PET/CT is a fairly effective study to locate the tumor lesion, and thus perform a surgical procedure.

ABSTRACT Background: Most data on the natural history of portal hypertension come from studies in adults. The morbidity rate of upper gastrointestinal bleeding (UGIB) in children with portal hypertension has not been systematically characterized. Objective: To describe the morbidity and mortality of UGIB in pediatric patients with portal hypertension and identify predictive factors for the occurrence of its main complications. Methods: This retrospective study included pediatric patients with cirrhotic portal hypertension or with extrahepatic portal vein obstruction (EHPVO). Mortality and UGIB complications within a period of up to 6 weeks of the bleeding were investigated. To determine the predictive factors of morbidity, a multivariate analysis was performed using logistic regression; all results were considered significant at P<0.05. Results: A total of 86 patients (51.2% with EHPVO and 48.8% with cirrhosis) had 174 bleeding events. Ascites was the most common complication (43.1% of all cases), being more prevalent in patients with cirrhosis (P<0.001). Cirrhosis was a predictor of the occurrence of any morbidity (OR 20.3). The need for blood transfusion was predictor of at least one complication (OR 5.8), ascites (OR 7.2) and infections (OR 3.8) in the general group and at least one complication (OR 11.3) and ascites (OR 5.8) in cirrhotic patients. The need for expansion was a predictor of any morbidity (OR 4.6) and infections (OR 3.9) in the general group, in addition to being predictor of infection in cirrhotic patients (OR 5.4). There were no deaths from UGIB in the six weeks post-bleeding. Conclusion: The study showed the relevance of morbidity after UGIB in pediatric patients with portal hypertension, especially in those with cirrhosis. The patients with hemodynamic instability requiring blood transfusion or expansion on admission are at increased risk of complications related to upper gastrointestinal bleeding and should be closely monitored.


RESUMO Contexto: A maioria dos dados sobre a história natural da hipertensão porta provém de estudos em adultos. A morbidade associada à hemorragia digestiva alta (HDA) em crianças com hipertensão porta ainda não foi sistematicamente estudada. Objetivo: Descrever a morbimortalidade da HDA em pacientes pediátricos com hipertensão porta e identificar fatores preditivos para a ocorrência de suas principais complicações. Métodos: Este estudo retrospectivo incluiu pacientes pediátricos com hipertensão porta cirrótica ou com obstrução extra-hepática da veia porta (OEHVP). A mortalidade e as complicações da HDA foram estudadas até seis semanas após o sangramento. Para determinar os fatores preditivos de morbidade, foi realizada análise multivariada por meio de regressão logística; todos os resultados foram considerados significativos com P<0,05. Resultados: Oitenta e seis pacientes (51,2% com OEHVP e 48,8% com cirrose) tiveram 174 eventos hemorrágicos. A ascite foi a complicação mais comum (43,1% de todos os casos), sendo mais prevalente em pacientes com cirrose (P<0,001). A cirrose foi preditor da ocorrência de pelo menos uma complicação (OR 20,3). A necessidade de transfusão sanguínea foi preditora de pelo menos uma complicação (OR 5,8), ascite (OR 7,2) e infecções (OR 3,8) no grupo geral e pelo menos uma complicação (OR 11,3) e ascite (OR 5,8) nos cirróticos. A necessidade de expansão foi preditor de qualquer morbidade (OR 4,6) e infecções (OR 3,9) no grupo geral, além de ser preditor de infecção em cirróticos (OR 5,4). Não houve óbitos por HDA nas 6 semanas pós-sangramento. Conclusão: O estudo mostrou a relevância da morbidade após HDA em pacientes pediátricos com hipertensão porta, principalmente naqueles com cirrose. Os pacientes com instabilidade hemodinâmica que necessitam de transfusão de sangue ou expansão na admissão têm risco aumentado de complicações relacionadas à hemorragia digestiva alta e devem ser monitorados de perto.

RESUMEN Antecedentes: Últimamente creció el interés en poder determinar, en etapas tempranas de las hemorragias digestivas bajas (HDB), aquellos factores de riesgo relacionados con la posibilidad de presentar resultados evolutivos adversos. Objectivo: Determinar los factores de riesgo asociados a sangrados graves, cirugía de urgencia y mortalidad hospitalaria. Material y métodos: Realizamos un estudio observacional y retrospectivo sobre 1.850 pacientes, atendidos en forma consecutiva entre enero de 1999 y diciembre de 2018 por HDB. Para evaluar el riesgo de gravedad analizamos trece variables en las primeras cuatro horas desde la admisión. Para determinar los factores relacionados con la cirugía de urgencia, agregamos la enfermedad diverticular y, para evaluar mortalidad, la cirugía de urgencia y el puntaje (score) preoperatorio de la Sociedad Americana de Anestesiología (ASA). Resultados: De los 1.850 casos, 194 fueron graves y 1656 leves/moderados. Resultaron estadísticamente significativos como factores de mayor gravedad: > 70 años, FC > 120 lat/min., TA < 90 mm Hg, oliguria, hematoquecia masiva, hematocrito < 30%, hemoglobina < 7 g/% y necesidad transfusional. Resultaron predictores significativos de cirugía de urgencia: > 70 años, anti-coagulación, hipotensión arterial, taquicardia, hemoglobina < 7 g/%, oliguria, transfusiones y hematoquecia masiva. Se construyó una fórmula pronóstica de requerimiento de cirugía (sensibilidad 94%, especificidad 74%, valor predictivo positivo 91% y valor predictivo negativo 81%). AUC: 0,89%. Fueron significativos para mortalidad: > 70 años, anticoagulados, hematoquecia masiva, transfusiones y cirugía urgente. De los dieciséis pacientes operados y fallecidos de la serie, quince presentaban un ASA ≥ IV. Conclusiones: Las variables utilizadas resultaron simples, fiables y estadísticamente significativas para predecir gravedad, cirugía de urgencia y mortalidad.


ABSTRACT Background: Background: There has been a growing interest in determining those risk factors associated with adverse outcomes in early stages of lower gastrointestinal bleeding (LGIB). Objective: The aim of our study was to analyze the risk factors associated with severe bleeding, emergency surgery and in-hospital mortality. Material and methods: We conducted an observational and retrospective study on 1850 patients consecutive managed between January 1999 and December 2018 for LGIB. We analyzed thirteen variables within the first four hours of hospitalization to evaluate risk severity. Diverticular disease was considered to determine factors associated with emergency surgery, and the preoperative American Society of Anesthesiologists (ASA) score was used to assess mortality and emergency surgery. Results: Out of 1850 cases, 194 were severe and 1656 were mild/moderate, Patients > 70 years, with HR > 120 beats/min, BP < 90 mm Hg, oliguria, massive hematochezia, hematocrit < 30%, hemoglobin < 7 g% and need for transfusions presented statistically significant associations with severe bleeding. Age > 70 years, anticoagulation, hypotension, tachycardia, hemoglobin < 7 g%, oliguria, need for transfusion and massive hematochezia were significant predictors of emergency surgery. A prognostic formula was constructed to predict the need for surgery (sensitivity 94%, specificity 74%, positive predictive value 91% and negative predictive value 81%). AUC-ROC: 0,89%. Age > 70 years, anticoagulation, massive hematochezia transfusions and emergency surgery were identified as predictors of mortality. Fifteen of the sixteen patients who underwent surgery and died had ASA ≥ grade 4. Conclusions: The variables analyzed are simple, reliable and statistically significant to estimate the risk of severe bleeding, need for emergency surgery and mortality.

ABSTRACT

La hemorragia digestiva determinada por una fístula entre manga gástrica y seudoaneurisma de arteria esplénica o polo superior de bazo es infrecuente. Se presenta un caso clínico de una paciente de 52 años con antecedentes de cirugía de manga gástrica y fuga anastomótica. Ingresó por hemorragia digestiva alta grave. Se operó de emergencia y realizó punto hemostático sobre cara posterior de manga gástrica. Se reintervino por resangrado realizándose la gastrectomía y esplenopancreatectomía distal por solución de continuidad de arteria esplénica. Dada la inestabilidad hemodinámica se efectuó un esofagostoma y yeyunostomía, reconstruyéndose a los 8 meses con buena evolución.


Gastrointestinal bleeding caused by a fistula between the gastric sleeve and a pseudoaneurysm of the splenic artery or upper pole of the spleen is uncommon. A clinical case of a 52-year-old patient with a history of gastric sleeve surgery and anastomotic leak is presented. She was admitted for severe upper gastrointestinal bleeding. She underwent emergency surgery and performed a hemostatic stitch on the posterior face of the gastric sleeve. She underwent reoperation due to rebleeding, performing gastrectomy and distal splenopancreatectomy due to discontinuation of the splenic artery. Given the hemodynamic instability, an esophagostomy and jejunostomy were performed, reconstructing at 8 months with good evolution.


O sangramento gastrointestinal causado por uma fístula entre a manga gástrica e um pseudoaneurisma da artéria esplênica ou pólo superior do baço é incomum. Apresenta-se o caso clínico de um paciente de 52 anos com história de cirurgia de manga gástrica e fístula anastomótica. Ele foi internado por hemorragia digestiva alta grave. Uma operação de emergência foi realizada e um ponto hemostático foi realizado na face posterior da manga gástrica. Foi reoperado por ressangramento, realizando gastrectomia e esplenopancreatectomia distal por descontinuação da artéria esplênica. Dada a instabilidade hemodinâmica, foi realizada esofagostomia e jejunostomia, reconstruindo aos 8 meses com boa evolução.

Introducción: La hemorragia digestiva alta representa una de las causas más frecuentes de morbilidad y mortalidad en los servicios de cirugía general y específicamente es la primera causa de mortalidad en el servicio, por lo que constituye una emergencia médico-quirúrgica. Objetivo: Identificar la existencia de un patrón estacional en la incidencia de hemorragia digestiva alta en época invernal y estimar la frecuencia de algunos factores de riesgo de esta enfermedad. Métodos: Se realizó un estudio descriptivo, de series temporales, de pacientes afectados por esa enfermedad que acudieron al Hospital Dr. Miguel Enríquez en el periodo comprendido entre diciembre de 2017 y enero de 2020. La muestra quedó conformada por 151 pacientes que presentaron como diagnóstico de ingreso hemorragia digestiva alta. Resultados: Predominó el sexo masculino y los mayores de 60 años. La temporada de mayor incidencia de esta complicación digestiva fue la invernal (diciembre, enero y febrero). Los factores de riesgo que predominaron fueron los hábitos tóxicos y la ingestión de AINES y ASA. La forma de presentación más frecuente fue la melena y la principal etiología la úlcera péptica duodenal. Conclusiones: Los casos con hemorragia digestiva alta predominaron en la temporada invernal y los factores de riesgo más frecuentes fueron los hábitos tóxicos y el uso de AINES en relación con el periodo estacional(AU)


Introduction: The High Digestive Hemorrhage represents one of the most frequent causes of morbidity and mortality in the General Surgery Services and it is specifically the first cause of mortality in our service. It constitutes a very frequent medical-surgical emergency. Objective: To determine the existence of a seasonal pattern in the incidence of HDA in winter and its relationship with risk factors. Methods: A descriptive and retrospective study was carried out with a longitudinal design in which patients affected by HDA were studied. These patients were assisted at "Dr. Miguel Enriquez Hospital" between December 2017 and January 2020. The study group was composed of all the patients who came to our emergency services with manifestations of bleeding from the upper digestive tract. The sample was made up of 151 patients who presented a diagnosis of HDA at the time of their admission. Results: The predominant sex was male and the age over 60 years old. The season with the highest incidence of this digestive complication was winter (December, January and February). The risk factors that predominated in our study were toxic habits and ingestion of AINES, ASA. The predominant form of presentation of the HDA were tarry stools, being the Duodenal Peptic Ulcer the main etiology. Conclusions: Cases with Upper Digestive Bleeding predominated in the winter season and the most frequent risk factors were toxic habits and the use of NSAIDs in relation to the seasonal period(AU)

Determinar la relación entre la infección por Helicobacter pylori y hemorragia digestiva alta (HDA) y su frecuencia fue el objetivo de la investigación, siendo ésta de tipo descriptivo correlacional con 50 pacientes con hemorragia digestiva, que acudieron al Servicio de Gastroenterología del Hospital Nacional Arzobispo Loayza de Perú, durante los meses de julio a octubre del 2021. Se elaboró un instrumento de recolección de información para las variables; edad, género, procedencia, ocupación y grado de instrucción. Se determinó la presencia de H. pylori a través de la realización de biopsia gástrica (identificación histopatológica). Para determinar la correspondencia entre las variables de la investigación, se utilizó la correlación de Pearson. Al evaluar el riesgo entre las patologías gastrointestinales y sufrir HDA se encontró para enfermedad ulcero péptica un (OR=7,1 IC: 1,394-35,987), gastritis (OR=13,7 IC: 1,068-174,81), y litiasis vesicular (OR=1,2 IC: 1,056-1,352). Dentro de los hábitos de los pacientes con HDA, 1 de cada 4 reportó consumir bebidas alcohólicas, 1 de cada 8 manifestó tabaquismo, y 1 de 2 reportó hábito cafeico. Se encontró una correlación positiva entre las variables hemorragia digestiva alta e infección por H. Pylori, ya que el coeficiente reportado es igual a 0,685. Se determinó una correlación positiva entre las variables hemorragia digestiva alta e infección por H. pylori en pacientes valorados por el servicio de gastroenterología del hospital. La infección por H. Pylori es de prevalencia elevada en la población general, sin embargo, el estudio muestra que no se manifiesta frecuentemente con sintomatología sangrante alta(AU)


Determining the relationship between infection by Helicobacter pylori and upper gastrointestinal bleeding (UGH) and its frequency was the objective of the investigation, this being descriptive correlational with 50 patients with gastrointestinal bleeding, who attended the Gastroenterology Service of the Arzobispo Loayza National Hospital. from Peru, during the months of July to October 2021. An information recollection instrument was developed for the variables; age, gender, origin, occupation and level of education. The presence of H. pylori was determined by performing a gastric biopsy (histopathological identification). To determine the correspondence between the research variables, the Pearson correlation was used. When evaluating the risk between gastrointestinal pathologies and suffering from UGIB, peptic ulcer disease (OR=7.1 CI: 1.394-35.987), gastritis (OR=13.7 CI: 1.068-174.81), and gallstones were found. (OR=1.2 CI:1.056-1.352). Among the habits of patients with HDA, 1 out of 4 reported consuming alcoholic beverages, 1 out of 8 reported smoking, and 1 out of 2 reported a coffee habit. A positive correlation was found between the variables upper gastrointestinal bleeding and H. Pylori infection, since the reported coefficient is equal to 0.685. A positive correlation was found between the variables upper gastrointestinal bleeding and H. pylori infection in patients assessed by the gastroenterology service of the hospital. H. Pylori infection is highly prevalent in the general population, however, the study shows that it does not manifest frequently with high bleeding symptoms(AU)

Resumen La hemorragia digestiva alta (HDA) es una emergencia médico-quirúrgica común que debe ser tratada precozmente por su alta morbimortalidad. Corresponde a sangrado del esófago, estómago o del duodeno proximal, y se divide en etiología no variceal y variceal. Dentro de las no variceales destaca la úlcera péptica como la más frecuente, siendo esta producida por un desbalance entre factores protectores y agresivos. Por otro lado, en las hemorragias variceales destacan las várices gastroduodenales, las cuales son consecuencia del aumento de la presión portal. La incidencia de la HDA a nivel mundial varía entre 37 a 172 casos por cada 100.000 habitantes por año y la mortalidad entre un 5 y un 14% según diferentes estudios. Lamentablemente, no existen cifras nacionales fidedignas de incidencia y prevalencia. El médico debe conocer bien la presentación clínica y la fisiopatología para ser asertivo en la sospecha, diagnóstico y manejo de esta patología. En cuanto al tratamiento, el enfrentamiento se divide en el manejo de urgencias y luego endoscópico, puesto que la resucitación temprana intensiva puede reducir la morbimortalidad en pacientes con HDA. A continuación se hará una revisión actualizada enfocada en los aspectos más relevantes del manejo de esta patología. Se obtuvieron los datos de Pubmed y Scielo, específicamente la búsqueda de artículos originales y de revisiones sistemáticas con las palabras "hemorragia digestiva alta", "úlcera péptica", "várices esofágicas" y otras relacionadas. Los criterios usados fueron artículos preferentemente menores a 5 años de publicación en revistas científicas de alto índice de impacto.


Upper gastrointestinal bleeding (UHD) is a common medical-surgical emergency that must be treated early due to its high morbidity and mortality. It corresponds to bleeding from the esophagus, stomach, or proximal duodenum, and is divided into non-variceal and variceal etiology. Among the non-variceal, the peptic ulcer stands out as the most frequent, being this produced by an imbalance between protective and aggressive factors. On the other hand, in variceal hemorrhages gastroduodenal varices stand out, are a consequence of increased portal pressure. The incidence of HDA worldwide varies between 37 to 172 cases per 100,000 inhabitants per year and mortality between 5 to 14% according to different studies. Unfortunately, there are no reliable national statistics of incidence and prevalence. The physician must have a good understanding of the clinical presentation and pathophysiology to be assertive in the suspicion, diagnosis, and management of this pathology. Regarding treatment, the confrontation is divided into emergency management and then endoscopic, because early intensive resuscitation can reduce morbidity and mortality in patients with UHD. This is an updated review which will be focused on the most relevant aspects of the management of this pathology. Data were obtained from Pubmed and Scielo, specifically searching for original articles and systematic reviews with the words "upper gastrointestinal bleeding", "peptic ulcer", "esophageal varices" and other related words. The criteria used were articles preferably less than 5 years old in scientific journals with a high impact index.

RESUMEN Introducción: la hemorragia digestiva alta constituye toda pérdida hemática de volumen suficiente como para producir hematemesis, melena o ambas, cuyo origen se encuentra entre el esfínter esofágico superior y el ángulo de Treitz. Objetivo: determinar los factores predictivos de mortalidad en pacientes con hemorragia digestiva alta. Métodos: se realizó un estudio observacional, analítico, de cohorte prospectiva. Se seleccionó una muestra de 456 pacientes mayores de 15 años con el diagnóstico clínico de hemorragia digestiva alta admitidos en el cuerpo de guardia de cirugía general y unidad de cuidados intensivos y emergentes del Hospital Celia Sánchez Manduley, Manzanillo, entre enero de 2020 a diciembre de 2021. Se identificó una cohorte de pacientes egresados vivos (n=415) y una cohorte de pacientes fallecidos (n=41). Se realizó un análisis bivariado y posteriormente un análisis multivariado. Resultados: el modelo más ajustado de los factores predictivos de mortalidad quedó constituido por las siguientes variables: edad mayor de 60 años, shock hipovolémico, insuficiencia cardíaca, insuficiencia hepática yhemorragia recidivante. Conclusiones: se obtuvo un modelo ajustado con los factores predictivos de mortalidad en pacientes con hemorragia digestiva alta. Por lo tanto, podremos egresar precozmente a un paciente con bajo riesgo de mortalidad, mientras que podemos considerar el ingreso de un paciente de alto riesgo en la unidad de cuidados intensivos.


ABSTRACT Introduction: upper gastrointestinal bleeding constitutes any blood loss of sufficient volume to produce hematemesis, melena or both, whose origin is between the upper esophageal sphincter and the angle of Treitz. Objective: to determine the predictive factors of mortality in patients with upper gastrointestinal bleeding. Methods: an observational, analytical, prospective cohort study was carried out. A sample of 456 patients older than 15 years with the clinical diagnosis of upper gastrointestinal bleeding admitted to the general surgery guardhouse and intensive and emergent care unit of the Celia Sánchez Manduley Hospital, Manzanillo, between January 2020 and December, was selected. 2021. A cohort of patients discharged alive (n = 415) and a cohort of deceased patients (n = 41) were identified. A bivariate analysis was performed and subsequently a multivariate analysis. Results: the most adjusted model of the predictive factors of mortality was made up of the following variables: age over 60 years, hypovolemic shock, heart failure, liver failure and recurrent bleeding. Conclusions: an adjusted model was obtained with the predictive factors of mortality in patients with upper gastrointestinal bleeding. Therefore, we can discharge a patient with low risk of mortality early, while we can consider the admission of a high-risk patient to the intensive care unit.


RESUMO Introdução: o sangramento gastrointestinal superior constitui qualquer perda de sangue de volume suficiente para produzir hematêmese, melena ou ambas, cuja origem se encontra entre o esfíncter esofágico superior e o ângulo de Treitz. Objetivo: determinar os fatores preditivos de mortalidade em pacientes com hemorragia digestiva alta. Métodos: foi realizado um estudo observacional, analítico e de coorte prospectivo. Foi selecionada uma amostra de 456 pacientes maiores de 15 anos com diagnóstico clínico de hemorragia digestiva alta internados na enfermaria de cirurgia geral e unidade de terapia intensiva e emergente do Hospital Celia Sánchez Manduley, Manzanillo, entre janeiro de 2020 e dezembro. coorte de pacientes que receberam alta com vida (n = 415) e uma coorte de pacientes falecidos (n = 41). Foi realizada uma análise bivariada e posteriormente uma análise multivariada. Resultados: o modelo mais ajustado dos fatores preditivos de mortalidade foi composto pelas seguintes variáveis: idade acima de 60 anos, choque hipovolêmico, insuficiência cardíaca, insuficiência hepática e sangramento recorrente. Conclusões: foi obtido um modelo ajustado com os fatores preditivos de mortalidade em pacientes com hemorragia digestiva alta. Por tanto, podemos dar alta precoce a um paciente com baixo risco de mortalidade, en quanto podemos considerar a admissão de um paciente de alto risco à unidade de terapia intensiva.

ABSTRACT BACKGROUND: The COVID-19 pandemic has changed digestive endoscopy services around the world. OBJECTIVE: This study aimed to measure the number of urgent/emergency endoscopic procedures performed in a Brazilian hospital, comparing it to the same period in the previous year, and to identify risk factors in COVID-19 patients undergoing endoscopic procedures for upper gastrointestinal bleeding (UGIB). METHODS: This was a retrospective, cross-sectional, observational, single-center study. The study evaluated urgent/emergency endoscopic procedures performed on adult patients from March to August in 2019 and 2020. The COVID-19 patients included were diagnosed using RT-PCR, aged over 18 years with complete medical record information. The variables evaluated were age, sex, comorbidities, length of stay, D-dimer, need for intensive care unit (ICU) and mechanical ventilation. Student's t-test for independent samples or the non-parametric Mann-Whitney test was used to compare quantitative variables. Categorical variables were analyzed using Fisher's exact test. A P-value <0.05 indicated statistical significance. RESULTS: A total of 130 urgent/emergency endoscopic procedures were performed in 2020 and 97 in 2019. During the study period, 631 patients were hospitalized due to COVID-19, of whom 16 underwent urgent/emergency endoscopic procedures, 10 (1.6%) due to UGIB. Of the variables analyzed, the need for ICU and/or mechanical ventilation during hospitalization was statistically significant as a risk factor for UGIB. CONCLUSION: This study showed increased urgent/emergency endoscopic procedures during the pandemic at the study site. Among the patients hospitalized with the novel coronavirus, there is a higher risk for UGIB in those needing ICU and/or mechanical ventilation.


RESUMO CONTEXTO: A pandemia da COVID-19 tem alterado o funcionamento de serviços de endoscopia digestiva pelo mundo. OBJETIVO: O presente estudo tem por objetivo medir o número de exames endoscópicos de urgência/emergência realizados em um hospital público brasileiro, comparando-o ao mesmo período do ano anterior, além de avaliar os fatores de risco dos pacientes com COVID-19 que realizaram exame endoscópico por hemorragia digestiva alta (HDA). MÉTODOS: Estudo retrospectivo, transversal, observacional e unicêntrico. Foram avaliados todos os exames endoscópicos de urgência/emergência realizados em pacientes acima de 18 anos, nos períodos de março a agosto dos anos de 2019 e 2020. Os pacientes com COVID-19 incluídos foram diagnosticados por RT-PCR, acima de 18 anos, com informações completas em prontuário. As variáveis avaliadas foram: tipo de exame endoscópico, idade, sexo, comorbidades, tempo de internação, d-dímero, necessidade de UTI e ventilação mecânica durante a internação. A comparação dos grupos em relação a variáveis quantitativas foi feita através do teste t de Student para amostras independentes ou o teste não-paramétrico de Mann-Whitney. As variáveis categóricas foram avaliadas pelo teste exato de Fisher. Valores de P<0,05 indicaram significância estatística. RESULTADOS: Observaram-se 130 exames endoscópicos de urgência/emergência no período avaliado em 2020, e 97 em 2019. No período do estudo foram internados 631 pacientes por COVID-19, dos quais 16 realizaram exame endoscópico de urgência/emergência, sendo 1,6% por HDA. Dentre as variáveis analisadas, necessidade de UTI e/ou ventilação mecânica durante o internamento foram estatisticamente significativos como risco para desenvolvimento de HDA. CONCLUSÃO: O presente estudo mostra que para a realidade local houve incremento de exames endoscópicos de urgência/emergência durante a pandemia. Dentre os pacientes internados com o novo coronavírus, há maior risco de HDA naqueles que necessitam de UTI e/ou ventilação mecânica.

Introducción: La hemobilia es por definición una causa de hemorragia digestiva alta, donde existe una comunicación de la vía biliar en cualquiera de sus segmentos con vasos sanguíneos que desembocan a través de la ampolla de Vater. Su presentación es infrecuente y no sospechada en la práctica clínica diaria de gastroenterólogos, cirujanos, hepatólogos, clínicos e intensivistas, con un difícil manejo diagnóstico-terapéutico y una elevada morbi-mortalidad. Objetivo: Describir tres casos de pacientes con diagnóstico de hemobilia. Desarrollo: Se presentan tres casos con hemobilia que tuvieron una elevada mortalidad y con diferente etiología; en el primer caso por trombosis de la arteria hepática postrasplante hepático, el segundo secundario a un colangiocarcinoma de la unión hepatocística y el tercero con diagnóstico de un aneurisma de la arteria hepática derecha confirmado y parcialmente tratado por angiotomografía, posteriormente intervenido quirúrgicamente y único sobreviviente. Conclusiones: Resultaron tres casos con hemobilia de diferentes causas, con una elevada mortalidad por la intensidad de la hemorragia digestiva alta y las comorbilidades asociadas, además de señalar que ninguno de ellos presentó la tríada clásica reportada por Quincke(AU)


Introduction: Hemobilia is, by definition, a cause of upper gastrointestinal bleeding, where there is a communication of the bile duct in any of its segments with blood vessels that flow through the ampulla of Vater. It is rare and it is not suspected in the daily clinical practice of gastroenterologists, surgeons, hepatologists, clinicians and intensivists, hence the diagnostic-therapeutic management is difficult and it has high morbidity and mortality. Objective: To report three cases of patients with a diagnosis of hemobilia. Case report: We report three cases of hemobilia of high mortality and different etiology. The first case had post-liver transplantation hepatic artery thrombosis, the second had asecondary cholangiocarcinoma of the hepatocystic junction and the third had diagnosis of confirmed right hepatic artery aneurysm partially treated by CT angiography, subsequently operated on and the only survivor. Conclusions: These three hemobilia cases had different causes, and high mortality due to the intensity of the upper gastrointestinal bleeding and the associated comorbidities, in addition to noting that none of them exhibited the classic triad reported by Quincke(AU)

Cada vez más los pacientes diagnosticados con anemia son referidos al gastroenterólogo para su evaluación. La necesidad de realizar un adecuado planteo clínico y una correcta interpretación de las pruebas de diagnóstico ha motivado la revisión de este tema. Varios trastornos gastroenterológicos, con frecuencia, conducen a anemia como resultado de pérdidas sanguíneas, inflamación, malabsorción o a consecuencia de las terapias farmacológicas. En algunas patologías como la cirrosis, EII o neoplasias las causas son a menudo multifactoriales. Esta revisión, pretende proporcionar un enfoque útil para la práctica clínica. Para ello se ha revisado la información actualizada acerca de la patogénesis, diagnóstico y tratamiento de la anemia vinculada a patologías digestivas y se han confeccionados cuadros y algoritmos para facilitar su comprensión.


More and more patients diagnosed with anemia are referred to the gastroenterologist for evaluation. The need to carry out an adequate clinical approach and a correct interpretation of diagnostic tests has motivated this review. Several digestive diseases frequently lead to anemia because of blood loss, inflammation, malabsorption, or drug therapies. In some of them such as cirrhosis, IBD or neoplasms, the etiology is multifactorial. This review is intended to provide a useful approach to clinical practice. To this aim, updated information on the pathogenesis, diagnosis, and treatment of anemia related to digestive diseases has been reviewed, and tables and algorithms have been built to favor its understanding.


Cada vez mais pacientes diagnosticados com anemia são encaminhados ao gastroenterologista para avaliação. A necessidade de realizar uma abordagem clínica adequada e uma interpretação correta dos testes de diagnóstico motivou a revisão deste tema. Vários distúrbios gastroenterológicos freqüentemente levam à anemia como resultado de perda de sangue, inflamação, má absorção ou pelas próprias terapias farmacológicas. Em algumas patologias como cirrose, DII ou neoplasias, as causas costumam ser multifatoriais. Esta revisão visa fornecer uma abordagem útil à prática clínica. Para esse fim, foram revisadas informações atualizadas sobre a patogênese, o diagnóstico e o tratamento da anemia associada à patologia digestiva e foram elaboradas tabelas e algoritmos para facilitar seu entendimento.

Introducción: El empleo de agentes antiagregantes plaquetarios se ve cada día más extendido en la población mundial. La decisión en relación al manejo endoscópico de este tratamiento debe estar basada en el tipo de indicación médica que justifica el uso de estos medicamentos. Se realizó una revisión de los artículos publicados en las bases de datos Pubmed, Scielo, Medline y Cochrane, relacionados con el tema. Objetivo: Profundizar en los conocimientos relacionados con la terapia antiplaquetaria periendoscópica digestiva. Desarrollo: Los antiagregantes plaquetarios son clave en el tratamiento y prevención de eventos trombóticos vasculares, cardíacos o cerebrales. Se han establecido las indicaciones de los antitrombóticos, también se ha evaluado el riesgo tromboembólico al suspender el tratamiento. El riesgo tromboembólico puede ser bajo o alto, por lo que se debe evaluar el riesgo - beneficio de suspender la terapia en este contexto. Todos los procedimientos endoscópicos presentan un riesgo de hemorragia, se considera el bajo riesgo de sangrado por debajo del 1 y alto riesgo por encima del 1 por ciento. La decisión acerca de la antiagregación en el periodo perioperatorio/ periprocedimiento, no solo depende del balance entre el riesgo trombótico y hemorrágico, sino también del tipo y la indicación del tratamiento antiplaquetario. Conclusiones: La antiagregación plaquetaria en pacientes que serán sometidos a procederes gastrointestinales intervencionistas se observa cada vez con mayor frecuencia. Esto hace necesario el conocimiento acerca de la valoración del riesgo de fenómenos trombóticos al interrumpir estos fármacos, junto con la determinación del riesgo potencial de sangrado según el procedimiento endoscópico realizado(AU)


Introduction: The use of platelet antiaggregant agents is increasingly widespread in the world population. The decision regarding the endoscopic management of this treatment should be based on the type of medical indication that justifies the use of these medications. A review of the articles published in the Pubmed, Scielo, Medline and Cochrane databases related to the subject was made. Objective: To deepen the knowledge related to periendoscopic digestive antiplatelet therapy. Development: Platelet antiaggregant are key in the treatment and prevention of vascular, cardiac or cerebral thrombotic events. Just as the indications of antithrombotic agents have been established, the thromboembolic risk has also been evaluated when treatment is suspended. The thromboembolic risk may be low or high, so the risk-benefit of discontinuing therapy in this context should be evaluated. All endoscopic procedures present a risk of bleeding considering the low risk of bleeding below 1 percent and high risk above 1 percent. The decision about anti-aggregation in the perioperative / periprocedural period depends not only on the balance between thrombotic and hemorrhagic risks, but also on the type and indication of antiplatelet therapy. Conclusions: Platelet anti-aggregation in patients who will undergo interventional gastrointestinal procedures is observed more and more frequently. This makes knowledge about the risk assessment of thrombotic phenomena necessary when interrupting these drugs, together with the determination of the potential risk of bleeding according to the endoscopic procedure performed(AU)

A hemorragia digestiva alta é emergência médica frequente e potencialmente grave. Pode ser causa varicosa e não varicosa. O objetivo deste trabalho foi avaliar as características clínicas, eficácia do tratamento, evolução e desfecho clínico de pacientes com cirrose hepática. É estudo retrospectivo comparativo com base nos prontuários médicos de pacientes que evoluíram com sangramento gastrointestinal alta de causa varicosa durante o primeiro semestre de 2018 e o de 2021. Em 2018 foram 44 (grupo 1) e em 2021 45 pacientes (grupo 2). Em conslusão, o perfil dos pacientes é em sua maioria de homens na 5ª. década de vida. A principal causa da cirrose foi o etilismo. A incidência varizes esofágicas aumentou e a de varizes gástrica se manteve. O tempo de internamento geral e em UTI e os encaminhamentos a UTI aumentaram, assim como o uso de todos os hemoderivados. O balão esofágico foi mais utilizado e mais efetivo na redução da mortalidade.


Upper gastrointestinal bleeding is a frequent and potentially serious medical emergency. It can have a varicose and non-varicose cause. The objective of this study was to evaluate the clinical characteristics, treatment efficacy, evolution and clinical outcome of patients with liver cirrhosis. This is a retrospective comparative study based on the medical records of patients who developed variceal upper gastrointestinal bleeding during the first half of 2018 and 2021. In 2018, there were 44 patients (group 1) and in 2021, 45 patients (group 2). In conclusion, the profile of patients is mostly men in the 5th. decade of life. The main cause of cirrhosis was alcoholism. The incidence of esophageal varices increased and that of gastric varices remained. The overall and ICU length of stay and ICU referrals increased, as did the use of all blood products. The esophageal balloon was the most used and most effective in reducing mortality.

ABSTRACT

Meckel's diverticulum can be present in up to 1.2% of the population. It is usually diagnosed as an imaging finding, but it can present with complications such as digestive bleeding, intestinal obstruction, diverticulitis, ulcers, and perforation, more frequently in childhood or infancy. The diagnosis workup for this condition will depend on their clinical manifestation, the most frequent being gastrointestinal bleeding of obscure origin or small intestinal bleeding. In this context, although capsule endoscopy is the preferred technique, its diagnostic yield for the detection of Meckel's diverticulum is not entirely clear and it has not been compared in a controlled studies with other diagnostic methods. Here we report the diagnosis of a Meckel diverticulum and its intestinal complications by means of capsule endoscopy in a patient with iron deficiency anemia and gastrointestinal bleeding


El divertículo de Meckel puede estar presente en el 1,2% de la población general. Usualmente es diagnosticado como un hallazgo, pero puede presentarse con mayor frecuencia en la niñez o infancia por sus complicaciones como hemorragia digestiva, obstrucción intestinal, diverticulitis, úlceras y perforación. El enfrentamiento diagnóstico de esta condición dependerá de la manifestación clínica, siendo lo más frecuente hemorragia digestiva de origen oscuro o de intestino delgado. En este contexto, si bien la cápsula endoscópica es la técnica de elección, su rendimiento diagnóstico para la detección del divertículo de Meckel no es del todo claro y no ha sido comparado de forma controlada con otras técnicas diagnósticas. En el presente caso se reporta el diagnóstico de un divertículo de Meckel y sus complicaciones intestinales mediante cápsula endoscópica en una paciente con anemia ferropriva y hemorragia digestiva.

Resumen Introducción: Con la escala de Glasgow-Blatchford (EG-B) se califica mediante datos clínicos, el riesgo de resangrado después de hemorragia del tubo digestivo alto (HTDA); y con las escalas de Forrest y Dagradi, mediante endoscopia. Objetivo: Evaluar la capacidad de la EG-B para identificar riesgo de resangrado a 30 días después de una HTDA; el estándar de oro de comparación fue la endoscopia. Método: Se analizaron 129 expedientes de pacientes con HTDA y endoscopia. Se cuantificaron las escalas de Glasgow-Blatchford, Forrest y Dagradi; se calculó sensibilidad, especificidad y área bajo la curva ROC (ABC-ROC) del riesgo de resangrado reportado por EG-B. Resultados: La EG-B identificó a 53 pacientes con riesgo bajo de resangrado (41.09 %) y 76 con riesgo alto (58.91 %). Con la endoscopia se identificó a 107 pacientes con hemorragia no variceal (82.94 %), 98 con riesgo bajo (89.9 %) y 11 con riesgo alto (10.09 %); además, 22 pacientes con hemorragia variceal (17.05 %), 12 con riesgo bajo (54.54 %) y 10 con riesgo alto (45.45 %). La EG-B mostró sensibilidad de 0.857, especificidad de 0.462 y ABC-ROC de 0.660. Conclusiones: La EG-B es sencilla, objetiva y útil para identificar riesgo de resangrado después de HTDA; se sugiere como herramienta de triaje en urgencias.


Abstract Introduction: The Glasgow-Blatchford scale (GBS) classifies the risk of re-bleeding after upper gastrointestinal bleeding (UGIB) using clinical data, whereas the Forrest and Dagradi scales do it by endoscopy. Objective: To assess GBS’s ability to identify re-bleeding risk within 30 days of an UGIB, using endoscopy as the gold standard for comparison. Method: 129 medical records of patients with UGIB and endoscopy were analyzed. The Glasgow-Blatchford, Forrest and Dagradi scales were quantified; sensitivity, specificity and area under the ROC curve (AUC-ROC) of GBS-reported re-bleeding risk were calculated. Results: GBS identified 53 patients with low re-bleeding risk (41.09 %) and 76 with high risk (58.91 %). Endoscopy identified 107 patients with non-variceal bleeding (82.94 %): 98 with low risk (89.9 %) and 11 with high risk (10.09 %); in addition, it identified 22 patients with variceal hemorrhage (17.05 %): 12 with low risk (54.54 %) and 10 with high risk (45.45 %). GBS showed a sensitivity of 0.857, specificity of 0.462 and an AUC-ROC of 0.660. Conclusions: GBS is simple, objective and useful to identify the risk of re-bleeding after UGIB; it is suggested as a triage tool in the emergency department.

Resumen La enfermedad celíaca tiene una considerable frecuencia en nuestro medio. La mayoría de los pacientes presenta mejoría clínica, serológica y endoscópica al poco tiempo de iniciada la dieta libre de gluten. Un muy bajo porcentaje puede presentar o desarrollar una "enfermedad celíaca complicada", entidad que comprende el esprue refractario, la yeyunitis ulcerativa y el linfoma intestinal, que conllevan pronósticos desfavorables, con requerimiento de tratamientos más radicales. Presentamos aquí el caso de un paciente de 77 años evaluado en nuestro centro, que ingresó para estudio de hemorragia digestiva aguda y se realizó finalmente diagnóstico de enfermedad celiaca complicada, requiriendo inicio de tratamiento con corticoides sistémicos y seguimiento estrecho ambulatorio.


Abstract Celiac disease is considerably frequent in our media. Gluten-free diet shows clinical, serological and endoscopic improvement in most patients shortly after its start. A few patients may present or develop a "complicated celiac disease", an entity that includes refractory sprue, ulcerative jejunitis and intestinal lymphoma, which carry unfavorable prognoses, requiring more radical treatments. We present here the case of a 77-year-old male patient evaluated in our center, who was admitted for study of acute gastrointestinal bleeding. Complicated celiac disease was diagnosed, systemic corticosteroids were started and a close follow-up was carried out.

RESUMEN Introducción: en los últimos años, se aprecia a nivel global un aumento del cáncer gástrico. La mayoría de los tumores gástricos primarios son malignos. En Matanzas, existe un incremento de esta patología. Objetivo: determinar el comportamiento clínico, endoscópico e histológico del cáncer gástrico diagnosticado. Materiales y métodos: se realizó un estudio observacional, descriptivo y prospectivo en el Departamento de Gastroenterología del Hospital "Dr. Mario Muñoz Monroy", de la ciudad de Matanzas, en el período de enero del 2017 a octubre del 2019. El universo fue 25 pacientes que presentaron cáncer gástrico por diagnóstico endoscópico e histológico. Resultados: el grupo de edad más afectado correspondió a los pacientes entre 61 y 70 años, (44 %). El sexo masculino predominó en un 68 %. Los factores de riesgo de mayor incidencia, fueron la dieta inadecuada y el hábito de fumar. Las manifestaciones clínicas más relevantes fueron: epigastralgia, plenitud gástrica y pérdida de peso. La variedad hística que predominó fue el adenocarcinoma difuso y la localización el antro. Conclusiones: el cáncer gástrico constituye un problema de salud que, al actuar sobre los factores de riesgo se puede disminuir su incidencia; con un diagnóstico precoz se logrará disminuir la mortalidad (AU).


ABSTRACT Introduction: an increase of gastric cancer is appreciated in the world in the last years. Most of the primary gastric tumors are malignant. There is an increase of this disease also in Matanzas. Objective: to determine the histological, endoscopic and clinical behavior of the diagnosed gastric cancer. Materials and methods: a prospective, descriptive and observational study was carried out in the Department of Gastroenterology of the Hospital "Mario Munoz Monroy, of Matanzas, in the period from January 2017 to October 2019. The universe were 25 patients presenting gastric cancer by histologic and endoscopic diagnosis. Results: The most affected age group was the one of patients among 61 and 70 years old (44 %). Male sex predominated in 68 %. The risk factors having higher incidence were an inadequate diet and smoking. The more relevant clinical manifestation were epigastralgia, gastric fullness and weight loss. The predominating tissue variety was the diffuse adenocarcinoma and antrum location. Conclusions: gastric cancer is a health problem the incidence of which could be reduced when acting on its risk factors; with a precocious diagnosis mortality will be reduced (AU).

ABSTRACT

Resumen La hemorragia digestiva alta se define como el sangrado originado en el tracto digestivo superior, proximalmente al ángulo de Treitz, y constituye la urgencia gastroenterológica más importante. Tiene una incidencia que varía, según el área estudiada, entre 48 y 160 casos por 100 000 habitantes y año. Aunque puede ser causada por numerosas etiologías, no debemos olvidar aquellas menos comunes, ya que pueden condicionar una alta mortalidad, como es el caso de la rotura de un aneurisma de la arteria hepática. A continuación, mostramos un caso representativo.


Abstract Upper gastrointestinal bleeding is defined as bleeding originating in the upper digestive tract proximal to the Treitz angle and is the most important gastroenterological emergency. Its incidence varies, depending on the area studied, between 48 and 160 cases per 100,000 inhabitants per year. Although it can be caused by numerous etiologies, we must not forget the less common ones such as a ruptured hepatic artery aneurysm since they can condition high mortality. We present a representative case.
